IMMUNIZATION REQUIREMENTS Every student entering kindergarten for the first time must have a completed health examination documented on the new State of Illinois Health form prior to enrollment. Proof of immunization must be documented by the physician at the time of the exam. The following vaccinations are recommended by age 2 and can be given in five visits to a doctor or public health clinic: *1 vaccination against measles/mumps/rubella (MMR) *4 vaccinations against polio *3 vaccinations against polio *4 vaccinations against diptheria, tetanus and pertussis (DPT) *3 vaccinations against hepatitis B *1 vaccination against varicella Booster doses of (DPT), polio, and measles,mumps rubella(MMR) are needed between ages 4 and 6. NEW DENTAL EXAM REQUIREMENT A law that was passed and will go into effect July 1, 2005, states that in all public and private schools, students in kindergarten, second grade, and sixth grade are required to have dental exams.

PHYSICAL EXAM REQUIREMENTS Children entering kindergarten or entering school for the first time are required to have a completed physical exam at the time of registration. A new physical exam form is available and must be used this year. LEAD SCREENING A Lead Screening Questionnaire should be completed for all children age 6 months to 6 years of age. If it is indicated a Lead Screening should be done. Lead screenings are done at the Marion County Health Department for $25. Medicaid is accepted for children 18 years of age and younger. A sliding scale is available for low income clients. An appointment is necessary. Immunizations must be up to date. Be certain that your child's physician reviews and confirms that all immunizations are up to date and documented on the physical exam form. The Health History section on the back of the form must be completed and signed by the child's parent or guardian.
